Site Evaluation Presentation

My psychiatry site visit was with Dr. Saint Martin in which I presented one of the patients I have been following for the first 2 weeks in my inpatient psychiatric unit. My patient was admitted for MDD with suicidal ideation. With my patient, he had progressed to a point where he was near admission. The plan throughout his admission was putting him on Burprorian and Vilazadone and continuing to observe him. When formulating the plan with my team, I learned Buproprian can actually be used to treat patients with a medical history of ADHD. We were able to hold his Myadias and gave him Burproian which helped control both his symptoms of ADHD and depression. Vilazadone is also a new drug that I discussed with Dr. Saint Martin which is a SSRI/SRA that works by inhibiting the reuptake of serotonin and partial agonist at the serotonin receptors. Something I was asked during my site visit was when should I schedule a follow up visit. This was a great question as patients with MDD have the highest risk of relapse the first week they are discharged. So it is vital that the follow up outpatient appointment should be scheduled within the first week so see how the patient is doing. 

For my final site evaluation I presented 2 H&P, drug cards and journal article. My first H&P discussed a case of schizophrenia in a relatively young female. Dr. St Martin had asked me if there are programs/health clinics that are designated for young adults with psychotic illnesses. I then discussed with him, Ontrack is a mental health clinic for such cases that patients may follow up on, but due to her location we had decided to follow up at BOOST, which offers similar help. My last H&P was similar to my first and discussed a patient with MDD who recently tried to commit suicide. Since Dr. St martin had educated me on the importance of early follow ups, I decided to do my journal article on a meta analysis thats main objective was to see post up inpatient, when the highest risk of suicide was. Just as Dr. St martin had discussed, patients 1 week post up had the highest risk when compared to 2-4 weeks and one month later. I was able to learn alot from my site visits from Dr. St Martin as Psych H&P differ from other specialties.
