My site evaluation for long term care was with Arti and it was a very great experience beyond just presenting my HPIs. For my HPIs, I first presented a case for a elderly woman following up for her BP. She was prescribed a blood pressure cuff at home and told to log her measurements before coming to the office. When going over her recent logs, her BP was controlled at home. Today in the office her blood pressure was elevated to 180/90, making us concerned whether or not she was taking her blood pressure medication. My patient confirmed that she takes her medication and also stated that the machine she has goes around his wrist. When I talked to the attending, we both discussed how those give you very fluctuating numbers when taking blood pressure and recommended she get another machine. We added another medication and wanted to follow up in a couple months. Arti had asked me questions pertaining to blood pressure in the elderly populations and how it is different then the general population.
What I enjoyed most about my site visit with Arti was how engaging she made it for us. For the first visit we went over a case study with her in which we were told to make ddx and further workup for a patient she had seen in the hospital. Allowing us to use our knowledge and investigate how we would have handled that situation. For our last visit, after we all presented and went over our drug cards, we also went over some PANRE questions, just to get familiarized with the material which I also appreciated very much. Overall, Arti had taught us a lot on our site visit and I was able to learn a lot in the short time of my site visits.
